2) get an eGPU if you consistently want to use both the external monitor and internal screen. I have found that in this way, my MacBook stays silent and the dGPU does not use max wattage. 1) use the MacBook only in clamshell mode. It is plenty powerful however the fans go jet mode when using both internal and external monitors. I have the 2019 16in and had sold my eGPU thinking the MacBook would be plenty powerful to use a 4k monitor. Running at full wattage generates extra heat which is why your fans ramp up. It starts running at full wattage (about 20W) when an external monitor and the internal monitor is used.
